Becoming a Patient
Avivo is primarily a referral-based clinic.
New patients for the following areas require a referral to be sent to our clinic by a General Practitioner (GP)/Family Doctor, Nurse Practitioner, Midwife, Specialist or Obstetrician (OB).
- Obstetrics & Maternity
- Gynecology
- Menopause
- Lactation/Breastfeeding Care – Please call our clinic at (403) 289-8996 prior to obtaining a referral so we can ensure you are seen as quickly as possible.
Referral Form: For convenience, Avivo has created an OPTIONAL referral form for providers that can be downloaded here.

Services not requiring a referral
The following services DO NOT require a referral to our clinic. Please call (403) 289-8996 to book an appointment.
*Circumcision consults are covered by Alberta Health Care; however, circumcision procedures are not.
**These services are not covered by Alberta Health Care. They are paid for directly by the patient or patients can use third-party benefits to cover their physiotherapy costs.
